1. About Us
Tooth Wagon is a mobile dental clinic delivering services to schools, daycares, and aged care facilities across Victoria. We provide general dental care under Medicare (CDBS) and private packages.
2. Bookings & Appointments
- Bookings can be made via our online form, email, or phone.
- Confirmation of your appointment will be provided once all required forms and consent are submitted.
- We reserve the right to reschedule appointments due to unforeseen circumstances (e.g., staff illness, vehicle issues, weather).
3. Eligibility for CDBS
Patients eligible under the Child Dental Benefits Schedule (CDBS) will receive services at no out-of-pocket cost for covered items. Proof of Medicare eligibility may be required.
If you are not eligible for CDBS, private fee packages will apply.
4. Consent
A signed consent form must be provided before any treatment begins. For minors or aged care residents, consent must be obtained from a parent, guardian, or legally responsible party.
5. Cancellations & Non-Attendance
Please notify us at least 24 hours in advance if you need to cancel or reschedule. Non-attendance may result in forfeiture of a time slot, and repeated missed appointments may impact future bookings.
